Integrated Care at RISE Clinic Nigeria

An Integrated Delivery System (IDS) is a “network of healthcare providers and organizations [that] provides or arranges to provide a coordinated continuum of services to a defined population, and is willing to be held clinically and fiscally accountable for the clinical outcomes and health status of the population served.”

RISE Clinic is a leader in integrated care in southeast Nigeria. Its model of coordinated care, in which all players within the healthcare delivery system (i.e. community health workers, primary care providers, disease specialists) communicate and play a unique role in the delivery of patient care, has proven to be a success in the region. This model centers on providing community members with the tools and unique incentives to support them in their role of driving the health and wellness of their own community.

RISE Clinic was designed to test the viability of the IDS model in Anambra State, Nigeria. Due to its success, RISE leadership is working to develop the Health Integration Project (HIP), to expand this model to existing medical facilities in the region.

Multilevel coordinated care provides patients a pathway to escalate through various care levels based on specific needs. At the community level, patients receive education, screening and support services to allow them to understand their health risks. High-risk patients are referred to primary health centers for confirmatory diagnosis and disease management. The teams work directly with patients, local primary health centers, and multi-party payers to devise a plan to help patients finance their continued care. Primary care providers determine the need to escalate patients to a specialty care level depending on the complexity of their condition. This coordinated care system maximizes the efficiency of available resources while optimizing outcomes.
